TREATMENT OF DEEP CARIES LESION AND STEPWISE EXCAVATION TECHNIQUE

Ăvod a cĂl: Sanace kariĂŠznĂ lĂŠze patĹĂ ve stomatologii jiĹž odedĂĄvna mezi zĂĄkladnĂ vĂ˝kony, avĹĄak i v dneĹĄnĂ dobÄ stĂĄle nenĂ zcela jednotnĂ˝ pĹĂstup k tomuto oĹĄetĹenĂ pĹĂpadÄ kazu blĂzkĂŠho zubnĂ dĹeni. ExistujĂ tĹi rozdĂlnĂŠ techniky oĹĄetĹenĂ, kterĂŠ majĂ svĂŠ vĂ˝hody nevĂ˝hody, nicmĂŠnÄ jejich spoleÄnĂ˝m cĂlem je zachovĂĄnĂ vitality zubu. PrvnĂ totĂĄlnĂ exkavace vĹĄech kariĂŠznĂch tkĂĄnĂ se zhotovenĂm definitivnĂ vĂ˝plnÄ, druhĂ˝m zpĹŻsobem selektivnĂ s ponechĂĄnĂm malĂŠho mnoĹžstvĂ zmÄklĂ˝ch pulpĂĄlnÄ vĂ˝plnÄ. TĹetĂ technikou sekvenÄnĂ exkavace, coĹž znamenĂĄ dvoudobĂŠ postupnĂ˝m odstranÄnĂm tkĂĄnĂ. Toto pĹehledovĂŠ sdÄlenĂ zamÄĹeno prĂĄvÄ na tĹetĂ typ tedy odloĹženou exkavaci moĹžnosti provedenĂ vyuĹžitĂm rĹŻznĂ˝ch remineralizaÄnĂch materiĂĄlĹŻ. Metody: Pro tento pĹehledovĂ˝ ÄlĂĄnek byly vyhledĂĄny studie anglickĂŠm jazyku za pomoci klĂÄovĂ˝ch slov (kaz blĂzkĂ˝ dĹeni, odloĹženĂĄ nepĹĂmĂŠ pĹekrytĂ dĹenÄ, hydroxid vĂĄpenatĂ˝, skloionomernĂ cement, kalciumsilikĂĄtovĂ˝ kalciumfosfĂĄtovĂ˝ cement) obmÄnou shrnout souÄasnĂŠ poznatky o metodÄ odloĹženĂŠ materiĂĄlech pouĹžĂvanĂ˝ch tĂŠto indikaci. VyhledĂĄvalo databĂĄzĂch PubMed, Web of Science Cochrane Library zdrojĂch tuzemskĂŠ literatury. Byly vybrĂĄny pĹedevĹĄĂm z ÄasopisĹŻ impakt faktorem vydanĂ˝ch rozmezĂ let 2010 aĹž 2020. hlubĹĄĂ pochopenĂ historickĂŠho vĂ˝voje problematiky byla literatura doplnÄna nÄkolik publikacĂ druhĂŠ poloviny minulĂŠho stoletĂ. ZĂĄvÄr: OĹĄetĹenĂ hlubokĂŠho zachovĂĄnĂm velmi diskutovanĂŠ tĂŠma postup, jak vĂ˝kon provĂŠst. PĹi dvoudobĂŠm jsou nepĹĂmĂŠmu dĹenÄ nejÄastÄji vyuĹžĂvĂĄny materiĂĄly bĂĄzi hydroxidu vĂĄpenatĂŠho skloionomernĂch cementĹŻ. KalciumfosfĂĄtovĂŠ cementy vzhledem ke svĂ˝m vlastnostem vysokĂ˝ potenciĂĄl vyuĹžitĂ indikaci zvýťenĂ ĂşspÄĹĄnosti ale jeĹĄtÄ tĹeba udÄlat podrobnÄjĹĄĂ in vitro vĂ˝zkum klinickĂŠ studie.
